Unified Community Outreach

Appendix 3: How PSR and CHAT Collaborate While PSR and CHAT serves distinct community needs, the programs collaborate to provide complementary services and improve overall system efficiency. Key areas of collaboration include:

  1. Coordinated Case Management Example Scenario: A CHAT team may encounter an individual with low-acuity medical conditions with a behavioral health component. CHAT can treat the medical need and refer to PSR Aftercare team for non-medical related follow-up care. Joint follow-up services can ensure a holistic approach to addressing the individual’s mental, behavioral, and physical health.
  2. Cross-Referral Systems Protocols exist which allow PSR and CHAT teams to refer cases to each other in real-time when a call better fits the other program’s scope. Example: If PSR responds to a wellness check that evolves into a low-acuity medical situation, CHAT can be activated for medical intervention.
  3. Shared Training Initiatives Crosstrain staff on trauma-informed care, crisis intervention, and basic medical or behavioral health triage. Encourage joint scenario-based training to simulate cases where PSR and CHAT teams collaborate, improving real-world coordination.
  4. Unified Community Outreach Conduct joint outreach efforts to educate the public on the distinct roles of PSR and CHAT, emphasizing when and how to seek their services. Collaborate on resource fairs and partnerships with community-based organizations to expand both programs' reach.
  5. Data and Performance Metrics Sharing Share operational data to identify trends and overlapping service needs, helping refine response models and inform resource allocation. Use joint data to build stronger cases for funding, demonstrating the combined impact of PSR and CHAT on reducing emergency services workload and improving community outcomes.
