Appendix 2: Community Health Assess and Treat (CHAT) M ISSION CHAT’s mission is to reduce low acuity medical calls and overdoses traditionally responded to by firefighters by assessing and treating in the field (rather than transport to emergency departments) with one paramedic and one emergency medical technician and then following up, when feasible and requested, within twenty-four hours. CHAT addresses the social and behavioral determinants of health barriers by directing community members to resources and educating them about available options aside from 911. PF&R has partnered with CareOregon to provide health care services to low-income Oregonians to address the dramatic increase in lower acuity medical calls across the region, by establishing CHAT response and follow-up units. The CHAT units respond to lower acuity medical calls across the city, with a focus in Southeast Portland and the downtown core area where substance use is most pronounced. CHAT Follow-Up Teams provide 24-hour post incident aftercare to reassess health-related social needs and assist navigation across the care continuum. The Follow-Up teams work to empower individuals to gain more independence over their own medical and social care, in part to relieve traditional First Response. CHAT program goals: • Change the system of health care delivery in pre-hospital care setting by responding to lower acuity medical calls, assessing, and treating in the field (rather than transport) and following up when feasible and requested within twenty-four (24) hours. • Connect/reconnect patients to healthcare providers through education to self- empower patients, decrease the burden on the 911 system, and protect the limited firefighter resources for higher-acuity needs. • Reducing the number of non-emergent calls from high utilizers. • Increase access to Medication for Opioid Use Disorder to enable individuals suffering from addition to pursue their own recovery. S COPE The CHAT units respond to lower acuity medical and overdose type code calls across the city, with a focus in Southeast Portland and the downtown core area where substance use is most pronounced.
